The Authorization for Automatic Withdrawal of Child Support Payments form applies to direct payors only.
To sign up for ACH Debit, complete the Authorization For Automatic Withdrawal of Child Support Payments Form* and attach a voided blank check. After the authorization form is received, completely filled out and signed by you, we will contact your bank to set up the automatic withdrawal.
Please allow 10 business days for automatic withdrawal to take effect. In the meantime, please continue to mail in your child support payments.
